Foster Care Case Managers provide intensive case management services to foster families, children and biological parents in coordination and collaboration with the referring agency and in compliance with state, Medicaid and agency standards; serve as an advocate and liaison for and on behalf of client. Case Managers are expected to comply with all licensure and contractual requirements so that the status of Foster Care program is not compromised. All NVFS employees are expected to demonstrate the organizational values and sensitivity to the diversity of the organization’s client base.

•Manage a caseload of specific clients. Develop and execute individualized treatment and service plans based on information gathered from assessments, evaluations, and observations.
•Conduct home visits to provide support in developing child’s relationship with the foster family in accordance with state, Medicaid and agency standards and requirements. Educate and counsel by guiding the child and developing supportive relationship with child/family to promote service plan. Complete documentation of home visits according to established schedule and narrative standards.
•Provide specialized services that may include:
?Assessments of family functioning and relationship in the foster home
?Counseling children and families in preparation for placement or discharge
?Assess periodically to determine the child’s needs for psychosocial, nutritional, medical, and educational services
?Supervise children in resource, foster, and adoptive homes.
?Serve as liaison with other community agencies and collateral service providers for service planning and coordination
•Coordinate services and referrals to minimize fragmentation of care, reduce barriers, and link children with appropriate services to ensure comprehensive, continuous access to needed medical, social, educational, and other services appropriate to the needs of the child. As needed, obtain appropriate services and resources. As required, collaborate the delivery of these services with other agencies (county, referring agencies, Family Assessment and Planning Teams) and providers involved with the child.
•Participate in the following activities as assigned: placement and discharge planning, ongoing training of foster parents, transporting clients as part of case planning, pager on-call rotation.
•Participate in regular supervision with Treatment Supervisor by providing regular updates and preparing case presentations.
•Ensure all client files including reports and pertinent documents are completed in accordance with program/licensing/state/Medicaid requirements and NVFS guidelines.
•Maintain current level of knowledge on program software and foster care licensing requirements.
Qualification Requirements: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. •Must have one of the following:
?Doctorate or Master’s degree in social work from a college or university accredited by the Council of Social Work Education or a field related to social work such as sociology, psychology, education or counseling, with a student placement in providing casework services to children and families. One (1) year of experience in providing casework services to children and families may be substituted for a student placement;
?Baccalaureate degree in social work or a field related to social work including sociology, psychology, education or counseling and one (1) year of experience in providing casework services to children and families;
?Baccalaureate degree in any field plus two (2) years’ experience in providing casework services to children and families.
•Prefer at least two (2) years related work experience in case management, child development, working with families and therapeutic services.
•Demonstrated proficiency in written and spoken language skills in English; bilingual Spanish skills in client languages preferred
•Proficiency in Microsoft Office Suite.
